Client Registration
We are here to help! Please fill out the registration form below and our Program Director will give you a call to discuss your needs and enroll you in the programs that are best suited to your situation.
IMPORTANT INFORMATION ABOUT OUR TRANSPORTATION PROGRAMS: As a non-profit organization with no government funding, our transportation supports are made possible entirely through private donations, fundraising, and community generosity. These programs are intended to support those who have either financial or logistical barriers to accessing treatment. We offer two options which can be used separately or together, based on your individual needs.
- Volunteer Driver Program: intended for individuals who have no other means of transportation to their cancer-related appointments. Cost is $25 per round trip. Financial assistance available, if necessary.
- Self-Drive Assistance Program: provides mileage support for those who have a family member or friend available to drive them but are experiencing financial strain. No cost to the client.
Choosing the program that best fits your situation helps ensure we can support as many clients as possible while prioritizing volunteer rides for those who have no alternative transportation.
